  • Salons
  • Grocery stores
  • Music events
  • Movies
  • And more

Residents can decorate their rooms to their tastes and invite friends and family over to visit to show off their independent lifestyle.

Adult Family Care

The AFC (Adult Family Care) Program is a MassHealth-funded program that provides medical and financial assistance to caregivers (typically family members) caring for a person in a family home setting.

The trained caregiver provides meals, personal care assistance, supports their activities in the community, and provides needed supervision.

Eligibility for AFC:

  • Is over the age of 16, and unable to live alone due to physical, emotional, or developmental disabilities.
  • Be MassHealth eligible (Standard or CommonHealth) with a physician’s written order.
  • Have a medical, physical, or cognitive condition that requires either hands-on or verbal daily assistance or supervision to complete one or more of the following activities: bathing, dressing, toileting, transferring between positions, ambulating, or eating.

Who can be an AFC Caregiver?

  • Parents, family members, friends - anyone 18 years or older, other than a spouse or legal guardian
  • Must be willing to share a home with, and be trained to provide care for, an individual with a qualifying disability
  • Must be willing to follow a “Plan of Care” that will be monitored by AFC staff
  • Must complete an application process, including an interview, CORI check, medical examination and home study.
  • Must also participate in yearly trainings, complete daily caregiver logs and participate in monthly home visits.

Shared Living

Supported by the Department of Developmental Services (DDS), Shared Living is when a person with disabilities lives with a non-disabled person who has accepted contractual responsibility for the well-being of the person.

It’s a person-centered model that allows people with disabilities to live in a qualified home within the community and receive care and support from a reliable, qualified person who will be consistent each day and provide continuity of care.

Cushing recruits, screens, trains, and delivers ongoing support to caregivers who meet the needs of the individual - and caregivers receive ongoing financial, behavioral, and nursing support under this model.

Through a shared living experience, Cushing has found that having one consistent care provider deepens the relationship between two people and strengthens access to the greater community.
